Service Improvement
This report by the OECD attempts to assess whether - and to what degree - better care coordination can improve health system performance in terms of quality and cost-efficiency. Coordination of care refers to policies that help create patient-centred care that is more coherent both within and across care settings and over time. Broadly speaking, it means making health-care systems more attentive to the needs of individual patients and ensuring they get the appropriate care for acute episodes as well as care aimed at stabilising their health over long periods in less costly environments. The Canadian Health Services Research Foundation and the Quality Healthcare Network are proud to announce Researcher on Call, a series of conference calls for healthcare researchers and decision makers designed to accelerate improvements in healthcare. Researcher on Call is an interactive series that brings researchers and decision makers together on a common topic. Each call is designed to clarify the findings in the selected research or synthesis discussed; identify and illustrate actions that can be taken on the findings; and explore the issues and challenges in fully implementing the findings in various settings. The dialogue will help participants consider the implications of the research results for improving their practice. The first Researcher on Call teleconference will focus on healthy healthcare workplaces and is scheduled for October 30, 2007. For more information or to register, click here.

Knowledge Improvement
This paper, in the Canadian Journal of Program Evaluation, describes the process of developing measures to assess knowledge exchange outcomes using the dissemination of a document about a “best practices in type 2 diabetes” as an example. A best practices model consists of knowledge synthesis, knowledge exchange (dissemination/adoption), and evaluation stages. This project developed a practical and usable tool to measure the reach and uptake of disseminated innovations. Click here to read more.
Quality Improvement
This report, chaired by Professor Sir John Tooke, focuses on the question of clinical effectiveness in UK. It is divided into 3 sections:
- a descriptive background of who is currently doing what in the UK
- current thinking on how to approach the issues of clinical effectiveness and barriers to implementation of guidelines or effective practices
- an overview of current knowledge on the effectiveness of strategies to promote the uptake of clinically effective practice.

Advancing the practice of patient- and family-centered care: how to get started
What is patient- and family-centered care? Why does it matter? How does it fit with our hospital’s overall mission? And finally, what can our hospital do to advance the practice of patient- and family-centered care? Where do we start? Today, hospital leaders and patients and families nationwide are asking these questions. The purpose of this document by the Institute for Family-Centered Care is to provide some answers. Event: “Author in the Room” - conference call with author of “Effectiveness of Teaching Quality Improvement to Clinicians”
The Institute for Healthcare Improvement (IHI) and The Journal of the American Medical Association (JAMA) are holding a conference call on Wednesday, 17th October, 2007, from 2pm – 3pm Eastern time (7pm UK time). The aim of this interactive conference call is to close the gap between knowledge – what is published in an article – and action – how much of this knowledge is put into practice to improve care. The interactive call will help readers consider the implications of the study results for improving their practice.
To help launch this unique collaboration between IHI and JAMA, IHI's Donald Berwick, MD, MPP, and JAMA Editor Catherine D. DeAngelis, MD, MPH, co-authored a JAMA editorial.

NPCi is a new NHS e-learning resource designed specifically for busy health care professionals and managers. The content covers prescribing, therapeutics and medicines management. Current therapeutics areas include: cardiovascular (including diabetes), CNS and mental health, common infections, pain management and respiratory tract conditions. Other areas are devoted to information mastery and medicines management — including developing people and organisations, general medicines management, improvement skills and tools, patient safety and risk, patients and their medicines, review of medicines, and service efficiency and reducing waste. The site also includes recorded workshops, quizzes to test your knowledge, regular blogs and podcasts and even the facility to join a discussion forum.
The developers are continuing to load content but would value your comments via their online feedback form on the existing subject matter so that adjustments can be made to the site before the wider launch early in 2008.

Quality Improvement
The House of Commons Public Accounts Committee has published a report on quality and safety in primary care. The report makes the following statements:
1. PCTs do not routinely include outcome measures for quality and safety in their commissioning arrangements.
2. One of the areas particularly associated with improving quality and safety was effective clinical leadership, yet members of PCTs’ Professional and Executive Committees often lacked effective leadership skills.
3. Not all GPs understand the concept of clinical governance and how it relates to their day to day work.
4. Patient and public involvement is less well developed than other aspects of clinical governance.
5. Of the 14 voluntary groups surveyed by the National Audit Office, all felt that PCTs could engage more effectively with their client groups.
6. Patients in primary care are often unclear about how to complain or how their complaints will be dealt with, whilst staff are not always informed of the outcome.
7. Only 4% of GPs report untoward events and clinical incidents to the National Patient Safety Agency’s National Reporting and Learning System resulting in limited sharing of learning either locally or nationally.

Improvement
System-level change requires three essential elements:
o The will to improve;
o Ideas about alternatives to the current state;
o The ability to execute change.

Link between health information technology and quality improvement
This report, from the Agency for Healthcare Research and Quality and co-authored by members of the Institute for Healthcare Improvement, looks at the link between health information technology and quality improvement in primary care.
